The delivery of hyaluronic acid, vitamins and antioxidants into the middle layer of the skin through fine needles. Around the eyes it supports hydration, fine lines and pigmentary under-eye darkening. It is performed in a clinical setting and patients return to daily life within a few hours.

What Is Mesotherapy?

Mesotherapy is a procedure in which vitamins, minerals, amino acids, hyaluronic acid and antioxidants are delivered into the middle layer of the skin through micro-injections. It has no role in treating eye disease. Around the eyes it is used as a supportive method for dryness, fine lines and changes in skin tone.

The solution is not a single fixed product. Its content varies with skin type, age and the nature of the complaint. At the consultation my first task is to establish whether the problem genuinely originates in the skin, because mesotherapy does not correct volume loss or herniated fat.

Under-Eye Mesotherapy: What It Can and Cannot Do

Under-eye mesotherapy aims to support the thin skin of the lower lid in terms of hydration, elasticity and colour. The question I am asked most often is whether it clears dark circles. The answer depends on what is causing them:

Pigmentary darkening: Caused by increased melanin in the skin. Mesotherapy can contribute meaningfully here, and tone usually lightens over several sessions.

Vascular darkening: Caused by the vessel network showing through thin skin. Mesotherapy can soften the appearance by supporting skin thickness and circulation, but will not remove it.

Shadow from hollowing: This is a volume problem, not a skin problem. Dermal fillers or NanoFat injections are the appropriate options.

Appearance created by bags: If fat has herniated, no injection corrects it; assessment belongs under lower eyelid blepharoplasty.

Sessions started without making this distinction end in disappointment. That is why I try to separate the source of the darkening with simple examination manoeuvres at the first visit.

Uses Around the Eyes

Hydration and Skin Quality

Hydrates dry, thin periocular skin and improves its resilience.

The hyaluronic acid content leaves the skin looking fuller and smoother.

Fine Lines

Softens the appearance of superficial wrinkles.

Supports collagen production and contributes to skin elasticity.

Tone and Radiance

Supports local circulation and reduces a dull appearance.

Helps even out tone where darkening is pigmentary.

Combined Planning

Where expression lines are prominent, it can be planned alongside botulinum toxin.

After surgery it can be used to support skin quality once healing is complete.

How Mesotherapy Is Performed

The procedure is carried out by a physician in a clinical setting. The area is cleaned first, and a topical anaesthetic cream may be used.

Very fine needles deliver small volumes into the superficial layer of the skin. Around the eyes both depth and distribution differ from the rest of the face.

A session usually takes 15-30 minutes.

A common protocol is 4-6 sessions at 2-4 week intervals, with spaced maintenance sessions afterwards.

Patients return to daily life within a few hours.

Who Is Suitable, and Who Is Not

Suitable candidates:

People with dryness, dullness and superficial fine lines around the eyes,

Those with pigmentary under-eye darkening,

Patients who prefer a gradual, non-surgical approach.

I postpone or avoid the procedure when:

There is active skin infection, eczema or an open wound in the area,

There is active blepharitis or uncontrolled allergic eye disease,

The patient is pregnant or breastfeeding,

The patient has a bleeding disorder or uses blood thinners, without first consulting the prescribing physician,

There is a known allergy to any component of the solution.

What to Expect Afterwards

Small blebs at the injection points settle within a few hours.

Mild redness and occasionally small bruises may appear and usually resolve within days.

No eye make-up for the first 24 hours.

Avoid sauna, steam rooms and intense exercise on the day of treatment.

Sun protection is the single habit that most affects how long the result holds.

The effect does not appear after one session; a noticeable change usually begins after the third. How long it lasts depends on skin type, age and sun protection habits.

The Limits of Mesotherapy

When I explain mesotherapy to patients, its limits are what I emphasise most. Mesotherapy does not:

Correct under-eye bags,

Replace significant volume loss,

Treat eyelid sagging or ptosis,

Erase deep expression lines on its own,

Treat any eye disease.

For this reason I treat it as one part of a periocular plan rather than a standalone solution. All the options are gathered on the periocular rejuvenation page.

Treatment in Istanbul

I am an ophthalmologist working in oculoplastic surgery in Kadıköy, Istanbul. I hold the FEBO diploma of the European Board of Ophthalmology and am an active member of ESOPRS and of the Oculoplastic Surgery Unit of the Turkish Ophthalmological Association.

Because mesotherapy is a multi-session protocol, patients travelling from abroad should know that a single visit rarely completes the course. In practice I plan the sequence around the patient's travel schedule, and where a full protocol is not feasible I say so at the outset rather than starting a series that cannot be finished.

When Should You See a Doctor?

If under-eye darkening appeared suddenly or affects only one side,

If it comes with swelling, pain or redness,

If there is itching and a constant need to rub — usually a sign of allergic eye disease,

If there is a new lump or colour change around the eye,

If there is flaking and redness at the lash line.

In these situations the cause needs to be established before any aesthetic procedure is started.

Frequently Asked Questions

What is under-eye mesotherapy?

Under-eye mesotherapy is the delivery of hyaluronic acid, vitamins and antioxidants into the superficial layer of the skin with very fine needles, to support the hydration, elasticity and colour of the thin lower lid skin. It is a skin treatment, not a treatment for any eye disease.

Does under-eye mesotherapy clear dark circles?

It depends on their source. Where the darkening is pigmentary, tone usually lightens over several sessions. Where it is vascular, the contribution is more limited. Where the darkness is shadow cast by hollowing, mesotherapy is not enough, because that requires volume.

How many mesotherapy sessions are needed?

A common protocol is 4-6 sessions at intervals of 2-4 weeks. The effect does not appear after a single session; a noticeable change usually begins after the third. Spaced maintenance sessions may be planned afterwards to hold the result.

Is mesotherapy painful?

Because very fine needles are used, discomfort is generally mild, and a topical anaesthetic cream can be applied beforehand. A session takes 15-30 minutes. The small blebs at the injection points settle within a few hours.

Does mesotherapy remove under-eye bags?

No. Under-eye bags are caused by forward displacement of orbital fat, and no injection into the skin corrects that. These patients are assessed under lower eyelid blepharoplasty. Mesotherapy is a treatment directed at the skin.

Who should not have mesotherapy?

It is not performed where there is active skin infection, eczema or an open wound in the area, or where there is active blepharitis or uncontrolled allergic eye disease. It is also avoided during pregnancy and breastfeeding, in patients with bleeding disorders or on blood thinners, and where there is a known allergy to any component.

When can I wear make-up after mesotherapy?

No eye make-up should be applied for the first 24 hours. Sauna, steam rooms and intense exercise should also be avoided on the day of treatment. Afterwards, using sun protection is the single habit that most determines how well the result holds.

How long does the effect of mesotherapy last?

Duration varies with skin type, age and sun protection habits, and it would be wrong to quote a fixed figure. It is not a permanent procedure and is maintained with spaced repeat sessions. Because the ageing process continues, a single course does not hold the result indefinitely.
